Program unit: Preparation for independent living. Child/family
planning: Surrogate parenting Transitional Care. SCHEDULE: Open year round
II. INSTITUTIONAL GROUP CARE CHILDREN CONSIDERED FOR
ADMISSION: Boys and girls, age 5-21 in DSS Custody; must be able to attend
public school or community college; must not be a danger to self or
others; we will consider mild to moderate emotional/behavior problems,
status offenders, and adjudicated delinquents. COTTAGES:
6 campus
cottages: groups of 6-10; live-in staff. EDUCATION: Grades 1-12
attend public schools with programs for EMD, LD, emotionally disturbed,
and behaviorally troubled. Also available - Vocational
education through community college, secondary vocational education,
technical and college training, on-campus library and learning center. RELIGION:
Attendance
required at weekly on-campus chapel service, non-denominational. RECREATION:
On-campus gym,
swimming pool, ball courts, Membership in local YMCA. SOCIAL SERVICES:
On-Staff Social workers. PSYCHOLOGICAL:
Testing,
counseling, therapy, and psychiatric services available in community. MEDICAL:
Pediatrician,
dentist, and other specialists in community. JOB TRAINING:
On-campus
job skills training program for individual children as appropriate; they
work, learn job skills, earn hourly wage, learn life skills. SPECIFIC RULES:
Must be 16 to date. Preparation for Adult Living youth may
own cars.

IV. FOSTER PARENT EDUCATION Model Approach to Partnership in Parenting (GPS/MAPP) training provided to all foster parents licensed through American Children's Home.

"Application for Service" required. Documentation required with
application: copy of court order, record of medical examination, social
history, birth certificate, psychological, education transcripts, Social
Security card, TB Tine test.
APPLICATION
PROCEDURE: Legal custodian should contact social worker by mail or phone.
Application is official when the completed and signed form is received.
Pre-admission visit is recommended for Transitional and PAL admission; child
must be present; Prefer to have family present
(Residential programs).
Written plans of service for all children admitted. DURING PERIOD OF CARE:
Review conferences every 3 months with family and referring agency. Visiting
families available. AFTER CARE: Services available for
residents and their families in conjunction with discharge plans. Consultation
services available.
